Angela's experience brings an enlightened view to even the most complex cases. For new patients: • Review health history • Diagnose • Develop tailored Treatment Plan Deductible, co-pay, co-insurance if applicable based on plan benefits. PLEASE NOTE: Due to Angela's limited availability, all treatment plans can/may be shared with associates in our office and you can book with any associate in our office.
Booking & Cancellation Policy
Service Bookings: NEW SERVICE FEE of $2.75 as of 7/1/2022 will be added to all appointments. Please note that fees are subject to change without prior notice. Please continue to WEAR A MASK in our office and in our building. Thank you for your compliance with the our building regulations and medical office setting to keep everyone safer. Thank you for supporting our small business. Cancellations: Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the provider's day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a $25 cancellation fee to the card on file.
